Patients have the right to receive copies of their medical records. You can request a copy of your medical records by mail or in person. You can request copies or a summary of the records. You may be charged a fee for costs related to medical record retrieval, copying and certification.
To obtain your medical records by mail:
You must print and complete the Authorization for Disclosure of Protected Health Information (PHI) form and mail request to:

Complete the consent form on-site in the Health Information Management Department at Grady. Health Information Management (Medical Records) is located on the first floor of Grady Memorial Hospital – Room E138.
