Request Medical Records

As a patient, it’s your right to get copies of most medical records.

Your medical record includes your health history, allergies, medicines, test results, and how to contact you. So, it’s our job to protect your privacy while giving you timely access when you need it.

Records and radiology images are stored electronically offsite. The state of Georgia regulates how much we can charge per page to cover the costs of labor, time, and resources needed to prepare your request. Grady complies with state and federal regulations related to the release of medical record information.

There are three ways to request medical records.

Use Our Medical Records Portal

You can request copies of your medical records and radiology images online. Complete the form and upload a copy of your driver’s license or state-issued photo ID. There is a $6.50 fee to receive the records electronically.

We recommend using either Chrome, Safari, or Firefox browser.

Use MyChart

MyChart gives you secure online access to your medical record. If you already have a Grady MyChart account, you can request a copy of your information by following these steps.

  1. Log in to MyChart.
  2. From the Main Menu, choose Document Center.
  3. Click on Requested Medical Records.
  4. Use the link to request your medical records. Select the information requested and add the visit date and any additional information.
  5. Click Submit Request.

It can take a few minutes to a few days to complete your request. The information appears on the Requested Medical Records page. From there, you can save it to your device.

In-Person, Mail, or Fax

You can also request your records and radiology images in person or by mail or fax. You’ll need to complete, sign, and date Grady’s medical records release form.

  • IN-PERSON: Visit Grady’s Patient Service Center, located on the Ground floor near the Main entrance. You will need a driver’s license or state-issued photo ID and the completed release form. If you are having someone else drop off your records request, they will need to show their driver’s license or state-issued photo ID and a copy of the patient’s (your) driver’s license or state-issued photo ID along with the completed release form.
  • MAIL: Mail a copy of your driver’s license or state-issued photo ID and the completed release form to:

  • FAX: Fax a copy of your driver’s license or state-issued photo ID and the completed release form to (404) 489-6447

After we process the request, we’ll mail you an invoice. Costs vary based on the number of pages released and the records requested. Once the invoice is paid, you will receive a copy of your records.

How does my insurance company get copies of my medical records to complete a claim?

Grady does not accept requests by phone. Insurance companies must submit record requests by fax to (404) 489-6447 or in writing to:

The request must include all supporting documents.

How can I submit a subpoena or discovery request for a medical record?

Subpoenas and discovery requests for medical records must be submitted to Grady, in writing, by mail, or in-person service. We cannot accept these types of requests electronically.

How do I get my medical records sent to another doctor?

You can use MyChart, use the steps outlined above or have the doctor’s office call Grady Medical Records at (404) 616-4282 to arrange for records to be transferred.
