If you live in Fulton or DeKalb county, you can apply and be evaluated for financial assistance. If you live outside Fulton or DeKalb county you come to a Grady facility for non-emergency or non-specialty outpatient medical care, you will be required to pay in full for your medical services before you will be seen by a healthcare provider. You can: Apply for financial assistance online, print out our application form and mail it to Grady Memorial Hospital Attn: Financial Assistance Program, or call (404) 616-1000 to schedule an appointment with a Financial Counselor.

MyChart gives you secure online access to your medical record. If you already have a Grady MyChart account, you can request a copy of your information by following these steps.

1. Log in to MyChart.
2. From the Main Menu, choose Document Center.
3. Click on Requested Medical Records.
4. Use the link to request your medical records. Select the information requested and add the visit date and any additional information.
5. Click Submit Request.

It can take a few minutes to a few days to complete your request. The information appears on the Requested Medical Records page. From there, you can save it to your device. Call (404) 616-5551 for assistance with MyChart.

To retrieve records in-person, visit Grady’s Patient Service Center, located on the Ground floor near the Main entrance. You will need a driver’s license or state-issued photo ID and the completed release form. If you are having someone else drop off your records request, they will need to show their driver’s license or state-issued photo ID and a copy of the patient’s (your) driver’s license or state-issued photo ID along with the completed release form.
You can also mail a copy of your driver’s license or state-issued photo ID and the completed release form to:

Finally you can fax a copy of your driver’s license or state-issued photo ID and the completed release form to (404) 489-6447.
After we process the request, we’ll mail you an invoice. Costs vary based on the number of pages released and the records requested. Once the invoice is paid, you will receive a copy of your records.

Grady does not accept requests by phone. Insurance companies must submit record requests by fax to (404) 489-6447 or in writing to:

The request must include all supporting documents.

Subpoenas and discovery requests for medical records must be submitted to Grady, in writing, by mail, or in-person service. We cannot accept these types of requests electronically.

You can use MyChart, use the steps outlined above or have the doctor’s office call Grady Medical Records at (404) 616-4282 to arrange for records to be transferred.
