Appointment Checklist
Before You Arrive
Before you arrive, verify where your appointment is at Grady Hospital, Correll Pavilion, or one of our Neighborhood Centers. You can find the location in your appointment reminder messages, MyChart, or call (404) 616-1000.
What to Bring
Please plan to arrive 20 minutes before your appointment. If you arrive after this time, you may be asked to reschedule. Please bring the following items to every appointment.
- Your photo ID
- Your health insurance plan card
- Your prescription insurance plan card
- Referral or authorization forms
- All current medications (prescription and over-the-counter) in their original containers
- Any outside medical records, X-rays, or prior test results not taken at this hospital (if your physician has requested them)
- Any questions you have for your doctor
- Follow ALL directions regarding food and drink before your appointment, or your test may be rescheduled.
Admissions
If you are being admitted, please bring only the essentials, including:
- Glasses
- Toothbrush and toothpaste
- Deodorant
- Robe
- Slippers
Personal care items such as contact lenses, eyeglasses, hearing aids, and dentures can be stored in your bedside stand when not in use. Please do not put them on your bed or food tray to help avoid them being lost or damaged. Leave valuables like jewelry or cash at home, or give them to a trusted relative or friend to watch over. Grady is not responsible for the loss or damage to patients’ belongings or valuables.
If you have an Advance Directive, please bring it with you when you visit the hospital or have an appointment with your doctor. A copy will be made and placed on your medical record so that we can know your wishes if we ever need to use it.

The Pratt Street pharmacy provides assistance with interactive telephone prescription refills, medication requests, Medicare Part D plans, and outreach services.

MyChart is a secure, online connection to your medical information all from the convenience of your tablet, smartphone, or computer. It removes barriers that keep patients from getting what they need from their healthcare provider. It’s all about access.

We want you to know and understand your financial responsibility for your visit or hospital stay; this includes insurance benefits and your responsibility for any deductibles, coinsurance, or co-payment at the time of service. Health Information Management (Medical Records) is located on the ground floor of Grady Memorial Hospital in our Patient Service Center– Room GA100. You can contact Medical Records to request copies or a summary of the records. Please note, you may be charged a fee for costs related to medical record retrieval, copying, and certification.

Pastoral Care

Goddard Chapel is open from 7:30 AM until 11 PM every day for prayer, worship, and quiet time. The chapel is located on the 1st floor. Chaplains are available 24 hours a day; seven days a week.
Our Interfaith Prayer Room is located near the chapel and always available. For access, contact the chaplaincy office or ask staff to page the duty chaplain.
Join us for weekly services Wednesdays at Noon or Sundays at 9:30 AM.
Photography & Video Use
Cellphone use for video or photography is not allowed during your hospital stay or visit. Additionally, cellphone use is not allowed when you are in the exam room with your doctor or during procedures. Please be courteous of other patients and families while using your phone in public areas.

Lost and Found
Grady’s lost and found office is located on the Lobby Level – Room GG015. The office is open Monday through Friday from 7 a.m. to 10:30 p.m.
