Patients have the right to receive copies of their medical records. You can request a copy of your medical records by mail or in person. You can request copies or a summary of the records. You may be charged a fee for costs related to medical record retrieval, copying and certification.
To obtain your medical records by mail you must print and complete the Authorization for Disclosure of Protected Health Information (PHI) form and mail request to:

You may also choose to complete the consent form on-site in the Health Information Management Department at Grady.
My Child
The parent or legal guardian must sign the consent release. Legal guardians will be asked to provide documentation of legal guardianship.
Patients who are 18 or older but unable to sign consent due to physical/mental illness
Durable Healthcare Power of Attorney (or other court appointed documentation) must be provided.
Patients who are deceased
Legal representation documentation must be provided.
Requests for copies will be processed within 15 business days; copies may be picked up in person, or mailed to the address provided.
Faxed requests are not accepted. However, we do accept faxes from doctor's offices and other hospitals.

Fees for personal records are as follows:
Exceptions include records needed for continuing care and worker's compensation claims (for first set of records only).
The Georgia Office of Planning and Budget has approved these fees for cost related to medical record retrieval, copying and certification effective July 1, 2004.
Certified copies of Birth and Death Certificates are available through the Georgia Office of Vital Records. You may complete an online request for a Birth Certificate or Death Certificate, or you may call the office at (404) 679-4702.
